A novel percutaneous intrafocal reduction technique for brachialis penetrating irreducible type 4 supracondylar humerus fracture

Type 4 supracondylar fractures are challenging to treat. Closed reduction may become impossible due brachialis muscle penetration and devastating results like a neurovascular injury that occur during recurrent manipulations. We here report an intra-focal percutaneous technique for maintaining closed reduction. An extensively posterior displaced type fracture with dimple at the antecubital fossa extensive ecchymosis is presented. During surgery, we could not obtain milking maneuver. inserted intrafocal K-Wire from side into site. With levering of wire, disappeared, after which maintained anatomical fixed two lateral K-wires. Neither complication nor residual deformity was observed postoperative follow-up. To show exact long-term effects safety this procedure, need more operated on by described technique.
